The Sangamon County Department of Public Health is seeking applicants to perform the duties of Service Coordinator for the Early Intervention Program to provide service coordination ensuring all qualified children under age 3 receive ongoing early intervention service delivery at the Child and Family Connections site for Region 18.
Requirements: Interested candidates must have a Bachelor's degree in human service, behavioral science, social science or a health related field OR a current State of Illinois registered nursing license. Candidates who have a current service coordinator credential is a plus. This position requires a valid Class D Illinois Driver's License, use of personal automobile and proof of auto insurance. Must be a Sangamon County resident or be willing to move within the County within six months of employment.
Skills: Computer skills are required.
Benefits: Work hours are typically Monday-Friday 8a-430p. Salary: $45,409/yr/$23.2867/hr; 2 weeks paid vacation, 3 paid personal days, 12 paid sick leave days and 12 paid holidays per year. Individual and Family health, dental and life insurance options, retirement and mileage reimbursement.
